Outcome Quality Measure

Outcome quality measures assess the results of care and services on a person’s health and/or quality of life, and whether the goals of care have been accomplished. Examples include the percentage of home health care episodes in which an individual’s ability to take their medications correctly has improved, or the percentage of hospital discharges that result in 30-day re-admissions.
