Enhanced Medical Home

Acronym: EMH

The EMH approach builds on the medical home model by placing additional requirements on the primary care provider and the health team, such as: targeted case management for enrollees at risk for high medical spending, increased access (e.g., 24/7 provider availability), integration of services such as nutrition interventions and mental health services, or coordination of care between subspecialists and the primary care provider team.14,15,16 An EMH might also emphasize the use of evidence-based protocols of chronic disease management (e.g., for asthma) or require the use of health information technology to improve the quality and coordination of care delivery.17 No organization currently accredits EMHs like those that provide accreditation for patient- centered medical homes.
